The short answer is, it sort of works. You'll have better results playing audio than video. Below are the steps to get it to work as well as I've been able to figure out.

First, you need to download and install a set of Real Media codecs from one of the following sites (you only need one of these).

Real Alternative (installs codecs only, not the whole hog): http://www.klcodecs.com/

(or http://home.hccnet.nl/h.edskes/finalbuilds.htm)
Other codec pack alternatives (I'm sure they are out there) www.google.com (http://www.google.com)
The latest Big Hog version of Real Player: www.real.com (http://www.real.com)
Older versions of the Real Player (less hoggy): www.oldversion.com (http://www.oldversion.com/program.php?n=real)

Next, test the Real Player codecs by playing some type of Real Media file (see Troubleshooting help below if you have problems).
Nuclear test video (http://www.nv.doe.gov/news%26pubs/photos%26films/0800070/0800070_hi.ram) (.ram, audio and video)
Crash test (http://www.nhtsa.dot.gov/NCAP/SideTest.rm) (.rm, video only)
The Razor's Edge (http://ramhurl.real.com/smildemohurl.html?file=smil/entertainment/razor_288_7/razor_288_7.smi) (.smi, audio and video)
DJ mixes (http://www.ravelinks.com/raveradio/djmixes.htm) (various, streaming audio)
http://www.bbc.co.uk/radio/
Other radio stations (http://beta.quinnware.com/forum/showthread.php?t=970)

Then, install a proper input plug-in for QCD.

The QCD video plug-in can handle some types of Real Media, and it comes with some of the more recent versions of QCD.
Some people have had better results using the Tara plugin. The Tara plugin for QCD (version 1.01) has recently been updated to work with QCD 4.50+. However, it is not the most stable plug-in and there seem to be issues with it playing video. Tara can be downloaded from www.musiclivesonline.com (http://www.musiclivesonline.com).
Paul started a plugin development for Real Media but is not able to finish it because of Real's terms. He has offered to give developers his existing code to finish the plugin here (http://www.quinnware.com/forum/showpost.php?p=10737&postcount=7).

Optionally, set up QCD as the default player for Real Media file types.
Do this in Preferences > Settings tab > System section > File Associations

Finally, test the Real Media playing in QCD by clicking on the test links above or dragging them into QCD.

This FAQ topic completely updated. Send me a PM if you have some other good links for testing Real Media. Some links for troubleshooting RM installations might also be helpful.

Thanks.
