Here are some documented symptoms
-QCD hangs a few seconds between tracks and there is no crossfade
-A track repeats continuously when QCD starts
-Poor audio quality when the DirectSound Crossfader is enabled

Whenever you are experiencing audio problems, it is a good idea to check for updates from the manufacturer of your audio card. And from Microsoft for your operating system. And for any other software or equipment that you think might be causing the problem.
